Have you heard of CAR? It is not what you think.

Cancer-associated retinopathy (CAR) is a rare, immune mediated paraneoplastic  syndrome.

[Paraneoplastic syndromes: Clinical manifestations due to indirect and remote effects produced by tumor metabolites or other products].

It is usually associated with small-cell carcinoma of the lung.

Cases have been reported with various systemic malignant tumours.

Specific autoantibodies are found against the photoreceptor protein recoverin. They cause apoptotic death of retinal photoreceptor cells.

Primary clinical presentation is rapid, progressive loss of vision and retinal degeneration that occur in association with  cancer without direct invasion or metastasis of cancer to the retina.

Other features : Concentric visual field defects, paracentral and mid-peripheral scotomas, night blindness, loss of colour vision, photosensitivity and pathological electroretinogram. There is also evidence of uveitis.

One should be aware of the signs and symptoms related to CAR as they can be the first manifestations of a hidden malignant tumour.
